If you are a victim of a crime/traffic offense and charges are filed in the City of Greeley Municipal Court you are encouraged to complete a Restitution/Victim Impact Statement.  This form must be filed with the City of Greeley Municipal Court. Click here to Download the Spanish version.

What is a Victim Impact Statement? A Victim Impact Statement allows a crime victim the opportunity to make a written statement regarding how the crime affected the victim physically, medically or emotionally. It additionally allows a crime victim to make certain sentencing recommendations to the court prior to the imposition of a sentence.

How do I request Criminal Restitution? You are entitled to request criminal restitution for actual and pecuniary damages sustained as a crime victim. This request must be accompanied by supporting documentation. The Restitution Request Form must be received by the City Attorney's Office (or the City of Greeley Municipal Court) prior to the defendant's first appearance in the City of Greeley Municipal Court (usually within 2-3 weeks from date of crime).  If you have any questions regarding the filing of a Restitution Request Form you may contact the City Attorney's Office.

Once the form is timely received by the City Attorney's Office and only if the defendant is ordered by the Court to pay criminal restitution the case will be scheduled for a restitution hearing. If all parties agree to the amount of criminal restitution that amount will be ordered by the Court. If a dispute arises regarding the amount of restitution that should be ordered the Court will conduct a hearing. The Court's Order will reflect the amount of restitution that the defendant will be required to pay as a condition of his/her sentence.

Victims of Greeley Municipal ordinance violations may also apply for victims assistance fee funds.  This is not an application for restitution.  You must also apply for restitution if charges have been filed against a defendant in your case.  This application must be completed within 30 days of crime occurrence.  This is a limited fund and expenses compensated must not be available through any other source.
